第一条 个人会员资格获得的条件

(一) 中华人民共和国境内的合法居民或遵守中华人民共和国各项法律的外籍人士。
(二) 提出加入本中心的申请
(三) 同意遵守本中心的章程
(四) 缴纳会费
(五) 经环球生命关怀健康管理中心同意接受，方可成为中心成员

第二条 个人会员资格的获得程序

(一) 提出加入本中心的申请并提供身份证明及相关个人信息
(二) 自愿接受健康管理中心的体检和测试
(三) 通过体检和测试，与上海环球生命关怀投资管理有限公司签订协议，并按照选择的服务类型缴纳相应费用，获得会员资格

第三条 获得个人会员资格应提交的材料
(一) 身份证明及相关个人信息
(二) 《环球生命关怀健康管理计划申请书》
(三) 缴费证明
(四) 通过体检和测试的证明
若该个人会员作为团体会员中的成员加入环球生命关怀计划，则同时应提供
(五) 团体会员资格证明
(六) 个人与团体的关系证明

第四条 个人会员资格的存续条件

(一) 已是个人会员
(二) 个人会员资格即将满期的两个月内
(三) 缴纳会费
(四) 经本中心同意接受继续成为会员

第五条 个人会员资格续期的程序

(一) 提出个人会员资格续期的申请并缴纳相应费用
(二) 通过环球生命关怀健康中心的同意

第七条 个人会员资格的结束

(一) 个人会员资格到期后未提出续期申请
(二) 个人会员身故
(三) 个人会员提出退出申请（仅在个人会员移民至海外）
(四) 未执行章程的相关规定，按规定应取消其个人会员资格
(五) 个人会员所属的团体会员提出撤销该个人会员资格

第八条 个人会员资格结束的程序
个人会员若是因为个人会员资格到期后未提出续期申请或因为未执行章程的相关规定，按规定应取消其个人会员资格的原因，则个人会员资格自然取消。
若是由于个人会员提出退出申请，则应遵照下面的程序：

(一) 个人会员提出退出申请（仅在个人会员移民至海外）
(二) 本中心接受个人会员的退出申请提供的资料包括：
1) 《环球生命关怀健康管理中心退会申请书》
2) 身份证明及相关个人信息
3) 海外移民（身份、居住、居留）证明

若是由于个人会员所属的团体会员提出撤销该个人会员资格，则应遵照下面的程序：

(一) 团体会员提出所属个人会员的退出申请
(二) 本中心接受团体会员的退出申请

提供的资料包括：
(一)《环球生命关怀健康管理中心退会申请书》
(二) 团体会员资格证明
(三) 团体会员与个人会员的关系证明

第九条 个人会员资格结束的会费处理

(一) 服务期限在一年以内的会员资格结束不作会费返还处理
(二) 服务期限超过一年以上的会员资格结束时，会费按照下面的计算公式计算返还： 返还金额=会费*60%*
（应服务时间-已服务时间）/应服务时间
(三) 不属于任何团体的个人会员，未满期会费返还本人或法定继承人
(四) 属于团体的个人会员，由团体确定未满期会费返还去向

第十条 团体会员资格获得的条件

(一) 中华人民共和国境内的合法团体，包括企业、事业单位、社会团体等
(二) 提出加入本中心的申请
(三) 同意遵守本中心的章程
(四) 缴纳会费
(五) 经环球生命关怀健康管理中心同意接受成为中心成员

第十一条 团体会员资格的获得程序

(一) 提出加入本中心的申请并签订协议
(二) 按照选择的服务缴纳相应费用，获得会员资格

第十二条 获得团体会员资格应提交的材料

(一) 资质证明
(二) 《环球生命关怀健康管理中心团体入会申请书》
(三) 缴费证明

第十三条 团体会员资格的结束

(一) 团体会员内所有的个人会员资格均结束后，团体会员资格自然结束
(二) 团体会员提出退出申请
(三) 未执行章程的相关规定，按规定应取消其团体会员资格
